2009 Dues Notification as of 9/10/2008
Due date: Dues and Renewal Forms are due by 12/1/2008
Late fee: A late fee of $25 applies to 2009 dues/fees not paid in full by 12/1/2008, unless you are joining as a first-time member after 12/1/2008.
Member payment plans: Payment plans are available for membership options, with a fee of $5 per payment. A late fee still applies if dues are not paid in full by 12/1/2008.
Directory Deadline: Directory changes and your renewal form must be received and processed prior to 1/15/2009 to be included in the 2009 Directory.

This form is for anyone who will be enrolled in their 3rd or 4th year of a NATAB, AUSTAB, or EUROTAB accredited Feldenkrais® Professional training program in 2009 and wishes to become an FGNA Member for the 2009 Membership Year.

2009 Special Rejoin Offer

Use this form if you have received an email offering a discounted $200 Professional Membership from FGNA (regularly $375 US/ $320 Canada). This rate is only valid for non-members who have not used the Rejoin rate before. This form combines our regular professional membership form with a provisional certification form, which is needed when your certification has lapsed.
